All About Flock Chiropractic Nutrition & Wellness Clinic

Since our founding in 1998, we have expanded greatly but still provide the same intimate, personal, and excellent service we did when we first opened. We at Flock Chiropractic Nutrition & Wellness Clinic, Inc. value you as a patient and appreciate that you have trusted us with your Chiropractic and wellness needs.   Please be advised that there are charges for each of the services that we provide to you as part of your treatment that your benefit plan may not cover.   Services include:  Chiropractic adjustments, massage/manual therapy, ultrasound, electrical stimulation, exercise, nutrition/wellness counseling.   Statements of Outstanding Balances are mailed at the end of each month.   If payment is not made or we do no hear from you after 90 days, your account will automatically be sent for collection.

Contracted Insurance Co.

As a courtesy, we will bill your insurance company for you.   Be aware that although our office may participate with your insurance company, coverage levels vary widely with each individual policy.   Your health insurance policy is a contract between you and your employer or insurance company.  We advise you to review your policy to educate yourself on what is covered and what is the patient responsibility.  When a claim is submitted by our office on your behalf, both the patient and the office will receive and EOB “explanation of benefits” in the mail.  The EOB details the treatment services rendered, the payment made for the services by your insurance plan and what your responsibility, if any, will be.  Payment for known patient portions (deductibles, copays) are due at the point of service.

 

Non-Contracted Insurance Co.

If our office is not contracted with your insurance company and can bill for “out of network” services, we would be happy to extend the same courtesy and submit billing on your behalf as stated above.   Please be advised that “out of network” coverage may result in a higher portion patient responsibility and we will provide you with a “good faith” estimate of your responsibility.  

All insurance benefits are verified by our staff to the best of our ability according to what is provided to us by your insurance company.   Please be advised that we may not be able to have access to your full benefit plan information and therefore cannot guarantee coverage of services – ie. massage benefits, deductible balance, and treatments rendered at other facilities.

Please bring your insurance card to your scheduled appointment. Failure to do so, will result in a self pay status for services rendered on that date of service and cannot be billed at a later time.

 

Point of Service Discount

If you do not have insurance coverage and/or wish to pay out of pocket, we offer a point of service discount on all services that must be paid on the same date the service was rendered.

Cancellation

If you are unable to keep an appointment, we ask that you provide us with at least 24 hours notice so that we can offer that appointment to another patient. A $70 no show fee will be charged if a patient does not show up for their scheduled appointment or provide sufficient notice.
